The perfect app and exactly what I needed. The ability to have an image upload is a must and we aren't sure why shopify doesn't include it. Will be using this for all of our forms from now on!

Great experience. I have had this app installed on one site for about a year and have had no problems. I installed it on another site and got hung up. Their support answered my request almost immediately late on a Friday. That was unexpected, but super awesome. They told me what I did wrong and offered to copy over the forms from the existing site to the new site. Top notch support on top of an app that works really well.
