Good app, support is quick to respond and help out. The app could be a tad more user-friendly but I assume these guys are just getting started. Overall, I recommend it, it's a cool custom form app for a Shopify store.

This app is great! We use it in several spots on our app and it runs flawlessly. The few times we have had any implementation issues, customer support was very responsive and resolved the issues. Highly Recommend!

Love this app! Helps me to stay organized and anytime I need help its always someone there to help me. Some of my forms disappeared from my site I chatted with someone and they fixed the problem in minutes!! Highly Recommend! Super user friendly!!!

So far so great. Sanjay is a rock star. Had a small problem and he jumped right in and helped me get it figured out.

Can not use custom domains to notify customers after form submissions. It confuses the customer and create distrust on a brand as they will receive it from some hulk formbuilder.com - Talked with support they are useless no solution. If that is what you are looking for not recommended at all.

Really enjoying this app, easy to use and build the form and set it up on your site. I'm glad I found this one! And specially the email support is very good. very quick response. Really Impressive.

I am Japanese. When I made an inquiry by email, I communicated in English, but what impressed me was how wonderful their customer support was. With a quick reply, he taught me carefully without stumbling over communication in English. And when I couldn't fix it, he fixed the code for me. A big thank you to Nisha for helping me out this time! Of course, the function of the app is also very good and easy to use! I would recommend it to anyone considering a form app.

Asked for a form to be set up, and after a few days of chatting, no forms we set up. Not an easy app to use for complex forms.

Really easy to install and customise to what I wanted. Had fonts to match the rest of my website. Just what I was looking for.

This Form Builder app has a great free version. it does exactly what I need - embedding any form I like.

Deleted the app from my store a while ago, but still trying to charge me a subscription. Did not have this problem with any other apps I deleted.

It's a good app. Customer Service is excellent. Credits can be removed only in paid plans. Adds some good custom features to the forms.

Fantastic form that allows you to upload images, with excellent customer service! Sarah went above, and beyond to help me with a slight issue I was facing that was solved very quickly. Keep up the excellent work! :D

The form is glitching when I scroll down or up, only after refreshing the webpage the customer can scroll down again.

This is a simple app to use and it integrates quite easily into the Shopify platform. Customer service is outstanding and really helpful.
