Super easy to use and to customize, great reporting functionality as well.

This is a great app. It's extremely useful and versatile. You can pretty much do anything you can think of with this app. The user interface is well built and easy to use. We were set up and building forms within 5 minutes. Honestly, the best part has been the support team. They contact you right back and get the issues resolved quickly. Super awesome app!

This is a great free form tool and we use it in multiple places on our site: Examples: https://slicklips.com/pages/recommend-store and https://slicklips.com/pages/wholesale The customization is awesome and it's also responsive, which is ideal since we get a lot of mobile visitors. At times, the loading speed is a touch slower than I'd like, but not enough to knock it down for a free app. Would probably deduct 1 star for that if it were a paid version, but even then it would still be a great product. Great job!

I spent 5 hours editing a form. I discovered a few things. 1. This is NOT an order form, it is a survey builder. 2. One wrong move and you have massive amounts of not-user-friendly editing. Lots and lots and lots of scrolling and copying and pasting to edit your mistake. 3. Little to no tutorial on how to create a form. 4. When the form is put in your shop, it is a tiny little space, and there is no amount of advanced CSS to make it bigger. I will be going back to my original idea of making a pdf and having my customers use that method.

Appears to working great. Wish there was a way to display it in a popup...

I looked at a number of form builder apps and this one appeared to provide everything I needed so I saw no need for one of the paid apps. It even supports conditional form objects - objects tht only appear based on teh vlaue the user chooses from a drop-down or whether a checkbox is clicked or not. As I used it I encountered one situation where I would like to have had a form object that wasn't supported - the ability to link to a url either via a text link or button. Other than that (and the need to not use an apostrophe anywhere) the app was flawless, and easy to configure with various post-submit options for what to do when the form is submitted. If you want a flexible, customizable form builder give this one a shot before paying for one of the competitors.

The best online form builder I've seen here, especially since it's free. The team is very helpful, fixed the minor glitches I've got on my website with this form. Awesome all the way!

The support team is very responsive and the form does exactly what it says on the package. Perfect solution if you want to use the same form for different language versions.

Great app, simple easy to use and super support.

Form is great, especially for being free. If the form could perform functions related to a Shopify account like creating customers it would be even better! As of right now, the form will send you by email all the data entered. There is no way to take that data from the form and create a new customer from it automatically.

Beautiful and easy to set up. However, despite working well for me, after going live with my site and these forms, not a single customer's "Submit" button has worked for them, and I have ended up manually receiving the form information from them via email. Will need to find another solution. It's disappointing because it all looks so good!

It took me a while to find a good app to build forms on our website, then I came across this app. It's free, customization is superb, tech support team is super responsive and no watermark at the bottom of the form. I'd recommend this app definitely!

From all the form builders this must be the best one. You can create endlesly forms on your website and the most important easy to customize, in any language and adding to your pages is easy as pie.

YOU CAN"T BEAT FREE!!! I wanted to create a form that customers could use to register for classes that we offer. I was able to create a form with information about our classes and allowed a customer to enter their name, email address, and choose from a set of offered classes. The software could use a bit of polishing to make it a little easier to use but its free. The submitted form is emailed to you and they are also store in the app's dashboard as a permanent record. Once your form is created, you need to copy and paste an html link into your page, pretty easy if you have a small amount of html knowledge. Very nice free app.

This app is great! Easy to integrate, many options to add and different type of forms, fully customizable and easy to use! I REALLY thank you guys! 6 out of 5 stars!

The app is intuitive, easy to use. The Auto Response e-mail confirmation for the customer is not working, which was the option that made me to install the app.

I was a bit hasty with my previous review and found a solution to one of the issues I highlighted, and a workaround for the other. Upgraded my review from 3 to 4 stars. In short, it's great app that is very useful. But because it sets such a high standard in its functionality, the small shortcomings are doubly disappointing.

Love it! I needed a form where users can upload images along with few information. This is the only app that gives image upload facility in absolutely "Free". Even though it is free app, they don't write any cheap label like "Powered by xxx" thats really cool.

The best form builder app and support team is great! If you need to create special forms, you should use this awesome app. PLUS! It's fully FREE!
