I don't often update a 5 star review, but this app deserves it. We've been using it since the day they launched and are so impressed with the features that have been added over the years. The customer service has always been top notch. I especially like the ability to create Shopify customers with fields linked to select Shopify fields (including accepts marketing!) Exporting the data is easy. I can't say enough good things!

This app gets customers to email me with their custom stencil requests... it works well - the forms are easy customizable and look great - I had a query and the customer service was fast and efficient.

Super easy to use, very interactive with loads of options and looks professional on my website. The support team is very fast which is a bonus.

The form we built using this app works well, except that it is incompatible with our email (Office 365), so we are unable to take advantage of having email confirmations look like they are coming from our own business. We are looking into work-arounds. Hulk Apps tech support was very responsive and friendly; they just weren't able to provide a workable solution.

I used the Forms app to create a number of forms on my organisation's website. The forms are easy to create and deploy and offer excellent functionality. When I encountered an issue, the support team were very quick in advising me how to resolve the issue. I have no hesitation in recommending this app to other potential users. =========== 10 July 2020=========== Since first using it, I have had a couple of queries that were very quickly resolved by the Hulk Apps Support team. Great work guys and girls! One thing I would love to see changed in the Form Builder is the ability to integrate PayPal payments as well as Stripe since it would give clients more payment options. ===== 22 Feb 2021 Had another issue with a new form I created. Once again, the support team stepped up to the plate and very quickly resolved the issue. Great support team!

Works ok but lots of downtime. I'm currently down on 3 websites now and can't offer support (over 8 hours now) and no ETA. Just getting a "formbuilder.hulkapps.com refused to connect" each time I try to load the app in any of the stores. Also a pita to get support with them, you have to create an account type of thing. 1 star for that. Update as of a week later* Still having issues with the apps, removed them to fix it and when I try to reinstall I'm actually seeing my old forms pop up for a second then I get an error from Shopify saying the page doesn't exist. So as of now I'm not only down a bit, but completely as of now for my forms.

Great app, very happy with the functionality, design and super-easy to set up! We used Form Builder to enable customers and artists to submit images, artwork, and provide feedback. Their customer service is amazing and they helped to implement our forms within 24 hours. Highly recommend their apps! Thank you Hulk Apps :)

Really great app, I am on the free plan (at the moment) but will probably upgrade when the time is right. I had a specific coding requirement and they fixed this for me very quickly with friendly service. I can't complain about anything! Thanks, Guy

I installed the product option form first and after continuously amazed by their service in helping me fixing all the issues - I decided to also replace my previous contact form with Hulk form builder. I enjoyed working with them and they are the best in terms of timely responding and solving your issues.

Easy to use . East to manage. Replaced old HTML table in 2 weeks. Forms are kind of old fashion but it did the job. Thanks.

We are using this for multiple sign up forms and image upload function via embedded forms and it works really well. The klaviyo integration is great and the support staff (Amit) are really quick and helpful.

Had a few little teething issues however resolved in less then 24hrs by Sanjay from the support team, very professional and friendly service!

My website is in French and it is impossible to write french without apostrophes (single quote character). It is also almost impossible to write English without single quotes. This app does not support single quotes in labels! When a developer does not bother to handle special characters in their UI, it makes you wonder about what other shortcuts they have taken. It is a shame as I like the app, on paper. It is well thought out and functionally rich. Uninstalling reluctantly

I am currently using the Free Version, A perfect app to create forms. Pick and drop sections, easy to Use. Spontaneous Customer Support. Recommended if you are looking for Customer feedback/ query forms.

App is nice but it doesn't have password field type in element. unable to create registration form with free version.

I love this app. I use it for a Trade Discount Form on 2 websites. They have provided awesome customer service.
