i really liked how easy to handle this app, i use this application to build a form in order to start sellling in Cash on Delivery. and it works very well, i recommed this app.

So far I really like it!! I like that it is easy to create the forms. Image upload is great! You do have to upgrade for that feature but the pricing is fair.

Great app works well

So far enjoying using the form builder, I am hoping to be able to make use of the free plan for our needs, but not sure if that is going to be possible yet.

We used the Pify Form on our shop, and are very happy with how the form works. All we needed was a simple form with radio buttons, text boxes and dropdown selections. This app has provided it all and more! Customer service is excellent too. Got a very quick respond after sending an email and the issue was solved in a matter of minutes. Would definitely recommend it.
