The form builder is a breeze to use. The ability to use conditional logic to show or hide various items on the form based on user input is also great, as is multi-step form options. The form has so many options for customization built in, as well as the ability to use custom CSS, so it can fit into nearly any site design. And, if you need help, the developer is also very responsive and quick to reply.

We highly recommend this app, it has worked so well for us and the support is amazing, everytime we needed something they fixed it right away.

I've looking for a Contact Us form that would replace the Dawn Theme default and this App is the perfect addition. The customer service agent Daniel helped me to setup the app as I wanted it on my store. Thank you Daniel!!

The software itself and the team's service are really great, but unfortunately, it's not compatible when accessed on a mobile device. I tried testing the Survey Form in the DEMO on my phone, but it couldn't display properly. It's such a shame.

I recommend this app. It is easy to use and does exactly what it is supposed to do. The support is fast and efficient.

This is the only form application that can send emails to other systems (e.g. CRM) where the sender is the email address from the email form field. A key thing for our CRM system. I highly recommend this app and am very happy with the fast responsive support.

I highly recommend this app because once I set it up last year, it has worked so smoothly that I forgot about it. I recently made some edits to my theme that caused an issue with the app. I contacted support and they fixed it right away. Thank you for the quick fix!

We have been using this app for a while. I added a new form, and it wasn't showing on the page for some reason. Support assisted and corrected the issue quickly.

Very easy to create, no developer needed. Great support team as well. The basic form is free and you can also upgrade to include Departments, Attached files, Auto-response, Thank you page redirect, an Info box, etc. Great price too.
