Ok so I've tried many apps on Shopify for my COD store and NONE of them worked as good as COD Monsters. There is NO app out there that offers you a free awesome theme + a fully customizable form with cross sells and upsells too!! It's an ALL IN ONE solution for your store for such a good price. Not to mention that their customer support team is freaking AWESOME!! They answered all of my questions and even helped with cutomizing the theme! Honestly, I'm impressed! I highly recommend this app! Kudos to the whole team!

i have been using the COD monster for more than 2 months . i have to say it , huge improvements it s so intuitive and easy to use , need more themes to be added but the conversion rate of the default theme is mind blowing no need to install more apps . you will have all in the same package.
